NJ Spotlight: Family Connects NJ Nurse Home-Visit Program Shows Early Impact

July 14, 2025 — New Jersey’s universal nurse home-visiting program, Family Connects NJ, is showing promising early results in improving maternal and infant health outcomes.

The program offers every family in participating counties a free visit from a registered nurse within the first few weeks after birth—regardless of income, insurance status, or immigration status. During the visit, nurses assess both parent and infant health, screen for postpartum depression and other risks, and connect families with community resources for ongoing support.

Initially launched in five pilot counties, Family Connects NJ has now expanded to 11 counties, with statewide availability expected in the coming years. Early evaluations show stronger links between families and health systems, increased access to early interventions, and improved follow-up care.

State officials and advocates say the program’s inclusive, preventive model is helping New Jersey address persistent disparities in maternal and child health. By meeting families where they are, Family Connects NJ is laying the groundwork for healthier starts and stronger communities.
